[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide athlete medical history management software for students within the district.
- Requirements of the proposed software include:
1. Must be able to run on Windows 7 and be cloud or web-based.
2. The vendor must provide technical support. The vendor must provide an 800-number that the end-user can call on an unlimited basis for questions about the software.
3. System must automate the secure collection and management process of the protected health information.
4. System must be accessible anytime and anywhere.
5. Must allow student athlete to enter protected medical information online.
6. System must allow for real-time updates for all users.
7. Emergency information of student athletes must be easily and quickly accessible.
8. System must allow for entry notes from athletic trainers onto student athletes’ profiles, including photographs of injuries if necessary.
9. System must allow for coaches to review their team roster and their clearance status.
10. System must indicate the completeness of each student athlete’s profile, alerting if there are incomplete documents.
11. Provider must not share any student athlete’s confidential or personally identifiable information to any third parties without prior written consent.
12. Mobile Apps are preferred.
13. Capability to add additional documentation (customized district forms) as needed.
(2) All questions had been submitted on November 9, 2018.
(3) Contract period will be for one year.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
